目的 通过汇总分析本中心临床药师参与的肿瘤患者营养会诊,旨在为促进肿瘤患者的合理营养支持提供参考依据,以期提高临床药师的会诊能力及药学服务质量。方法 选取本中心2021年2月至2022年2月临床药师营养会诊的70例肿瘤患者,运用统计学方法分析比较患者营养风险筛查2002(nutrition risk screening,NRS 2002)、患者主观整体评估(patient-generated subjective global assessment,PG-SGA)评分、实验室检查、人体测量指标、24 h膳食回顾调查及个体化营养干预方案。结果 77.14%的会诊患者存在营养风险,以头颈部肿瘤、消化道肿瘤和肺癌居多,有无营养风险、不同PG-SGA分级(A、B、C)患者体质指数(body mass index,BMI)和24 h膳食量存在统计学差异,营养干预方案中饮食+口服营养补充(oral nutritional supplement,ONS)占比最高(35.71%),其次为饮食指导+营养教育(21.43%)与全肠外营养(total parenteral nutrition,TPN) (20.00%),个体化肠外营养支持中全合一营养液(all in one,AIO)占89.66%。结论 肿瘤患者营养风险和营养不良发生率较高,临床药师合理选择筛查工具,重视肿瘤患者营养状况的动态评估,为其提供个体化的营养治疗方案。
Abstract
OBJECTIVE To summarize and analyze nutritional consultations of cancer patients participated by clinical pharmacist, provide reference for promoting suitable nutritional support for cancer patients and improve the consultation ability of clinical pharmacist and the quality of pharmaceutical care. METHODS A total of 70 cancer patients underwent nutritional consultations with clinical pharmacist from February 2021 to February 2022 in the hospital were selected, NRS 2002, PG-SGA, laboratory examinations, anthropometric indices, 24 h dietary survey and individualized nutrition intervention programs were collected and statistic analyzed. RESULTS The 77.14% of the patients were at risk of malnutrition, most of which were head and neck neoplasms, gastrointestinal tumors, and lung cancer. There were significant differences in BMI and 24 h dietary intake among patients with or without nutritional risk and patients with different grades of PG-SGA (A, B, C). In the nutritional interventions, dietary+ONS accounted for the highest proportion (37.5%), followed by dietary guidance + nutrition education (21.43%) and TPN (20.00%), AIO accounted for 89.66% of individual parenteral nutrition support. CONCLUSION The prevalence of malnutrition is high in patients with cancer. Clinical pharmacist chooses screening methods properly, attach importance to the dynamic assessment of the nutritional status of cancer patients, and provide them with individualized nutritional treatment.
